2. Application for membership
Please note that you must apply for National Health Insurance at the Toyohashi City National Health Insurance and Pension Division or the Counter Center within XNUMX days if any of the following applies.
・ When a child is born (for children)
・ When you join the National Health Insurance in another town and register as a resident in Toyohashi City when you move.
・ When you quit your workplace health insurance due to retirement
・ When you quit your workplace health insurance due to the retirement of your family
・ When you no longer receive welfare
3. Withdrawal procedure
Please note that you must complete the withdrawal procedure for National Health Insurance at the Toyohashi City National Health Insurance and Pension Section or the Counter Center within XNUMX days if any of the following applies.
・ When moving to another town
・ When you have health insurance at work
・ When you take out workplace health insurance due to employment of your family
・ When a person who has taken out National Health Insurance dies
・ When receiving welfare
